The neural system provides point-to-point rapid coordination among organs, but neural fibres do not innervate all cells and cellular functions need continuous regulation. A special kind of coordination and integration is provided by hormones. The neural and endocrine systems jointly coordinate and regulate the physiological functions of the body. Neural system fast hai par har cell tak nahi pahunchta — isliye hormones ka coordination chahiye. Endocrine glands aur hormones

Endocrine glands lack ducts and are called ductless glands; their secretions are hormones. Classically, a hormone is a chemical produced by endocrine glands, released into the blood and transported to a distantly located target organ. The current definition: hormones are non-nutrient chemicals which act as intercellular messengers and are produced in trace amounts. Invertebrates have very simple endocrine systems with few hormones; vertebrates have a large number of chemicals acting as hormones. Endocrine glands = ductless; secretion = hormones. 19.2.1The hypothalamusहाइपोथैलेमसHypothalamus

The hypothalamus is the basal part of the diencephalon (forebrain) and regulates a wide spectrum of body functions. It has several groups of neurosecretory cells (nuclei) that produce hormones which regulate the synthesis and secretion of pituitary hormones. They are of two types: releasing hormones (stimulate pituitary secretion — e.g., gonadotrophin releasing hormone, GnRH, stimulates synthesis and release of gonadotrophins) and inhibiting hormones (e.g., somatostatin inhibits release of growth hormone). These hormones pass through axons, are released from nerve endings and reach the pituitary through a portal circulatory system, regulating the anterior pituitary. The posterior pituitary is under direct neural regulation of the hypothalamus. Hypothalamus = diencephalon ka basal part. 19.2.2The pituitary glandपीयूष ग्रंथिPituitary gland

The pituitary lies in a bony cavity, the sella tursica, attached to the hypothalamus by a stalk. It has an adenohypophysis (pars distalis + pars intermedia) and a neurohypophysis (pars nervosa). The pars distalis (anterior pituitary) produces growth hormone (GH), prolactin (PRL), thyroid stimulating hormone (TSH), adrenocorticotrophic hormone (ACTH), luteinizing hormone (LH) and follicle stimulating hormone (FSH). The pars intermedia secretes melanocyte stimulating hormone (MSH); in humans it is almost merged with the pars distalis. The posterior pituitary stores and releases oxytocin and vasopressin, which are synthesised by the hypothalamus and transported axonally. Pituitary sella tursica mein. 19.2.3The pineal glandपीनियल ग्रंथिPineal gland

The pineal gland is on the dorsal side of the forebrain and secretes melatonin, which regulates the 24-hour (diurnal) rhythm — e.g., the normal sleep–wake cycle and body temperature. Melatonin also influences metabolism, pigmentation, the menstrual cycle and our defence capability. Pineal → melatonin → 24-hour rhythm (sleep-wake, temperature), metabolism, pigmentation, menstrual cycle, defence.

19.2.4Thyroid glandथायरॉइड ग्रंथिThyroid gland

The thyroid has two lobes on either side of the trachea, joined by a thin flap of connective tissue, the isthmus. It is made of follicles and stromal tissue; follicular cells synthesise tetraiodothyronine (T₄, thyroxine) and triiodothyronine (T₃). Iodine is essential for normal thyroid hormone synthesis. Iodine deficiency → hypothyroidism and enlargement of the thyroid = goitre. Hypothyroidism during pregnancy → defective development of the baby, cretinism (stunted growth, mental retardation, low IQ, abnormal skin, deaf-mutism). In adult women it may cause irregular menstrual cycles. Cancer or nodules of the thyroid → abnormally high hormone levels (hyperthyroidism); exophthalmic goitre (Graves’ disease) — enlarged thyroid, protruded eyeballs, increased BMR, weight loss. Thyroid hormones regulate basal metabolic rate, support RBC formation, control carbohydrate, protein and fat metabolism, and maintain water and electrolyte balance. The thyroid also secretes thyrocalcitonin (TCT), a protein hormone that regulates blood calcium. Thyroid: 2 lobes + isthmus; follicles → T₄, T₃ (iodine zaroori). Iodine kami → goitre; pregnancy mein → cretinism. Zyada → hyperthyroidism, Graves’ disease (exophthalmic goitre). Kaam: BMR, RBC, metabolism. TCT → blood Ca²⁺.

19.2.5Parathyroid glandपैराथायरॉइड ग्रंथिParathyroid

Humans have four parathyroid glands on the back of the thyroid, one pair in each lobe. They secrete a peptide hormone, parathyroid hormone (PTH), regulated by circulating calcium. PTH increases blood Ca²⁺: it stimulates bone resorption (dissolution/demineralisation), reabsorption of Ca²⁺ by renal tubules and Ca²⁺ absorption from digested food. PTH is thus a hypercalcemic hormone; along with TCT it maintains calcium balance. 4 parathyroids → PTH → blood Ca²⁺ badhata (bone resorption, kidney reabsorption, gut absorption) = hypercalcemic. TCT ke saath Ca balance.

Thymus

The thymus is a lobular structure between the lungs, behind the sternum on the ventral side of the aorta, and plays a major role in developing the immune system. It secretes peptide hormones, thymosins, which help differentiate T-lymphocytes (cell-mediated immunity) and promote antibody production (humoral immunity). The thymus degenerates in old individuals, so thymosin production falls and immune responses weaken. Thymus (sternum ke peeche) → thymosins → T-lymphocytes, antibodies. Budhape mein thymus chhota → immunity weak.

19.2.7Adrenal glandअधिवृक्क ग्रंथिAdrenal gland

A pair of adrenal glands sits on the anterior part of each kidney. Each has a central adrenal medulla and an outer adrenal cortex. The medulla secretes adrenaline (epinephrine) and noradrenaline (norepinephrine) — catecholamines — rapidly released during stress and emergencies: the emergency hormones / hormones of Fight or Flight. They increase alertness, pupillary dilation, piloerection (raising of hairs), sweating, heartbeat, strength of heart contraction and rate of respiration; they stimulate glycogen breakdown (raising blood glucose) and breakdown of lipids and proteins. Adrenal = kidney ke upar. Medulla → adrenaline, noradrenaline (catecholamines) = fight or flight: alertness, pupil dilation, sweating, heartbeat, saans badhe; glycogen breakdown.

The adrenal cortex has three layers — zona reticularis (inner), zona fasciculata (middle) and zona glomerulosa (outer) — and secretes corticoids. Glucocorticoids (main: cortisol) regulate carbohydrate metabolism: stimulate gluconeogenesis, lipolysis and proteolysis, inhibit cellular uptake and use of amino acids; cortisol maintains the cardiovascular system and kidney functions, produces anti-inflammatory reactions, suppresses the immune response and stimulates RBC production. Mineralocorticoids (main: aldosterone) act on renal tubules: reabsorption of Na⁺ and water, excretion of K⁺ and phosphate — maintaining electrolytes, body fluid volume, osmotic pressure and blood pressure. Small amounts of androgenic steroids are also secreted (role in axial, pubic and facial hair at puberty). Underproduction of cortex hormones alters carbohydrate metabolism, causing acute weakness and fatigue — Addison’s disease. Cortex: zona reticularis / fasciculata / glomerulosa. Glucocorticoids (cortisol): gluconeogenesis, anti-inflammatory, immunity dabaye, RBC. Mineralocorticoids (aldosterone): Na⁺ + paani reabsorb, K⁺ bahar → BP. Kami → Addison’s disease.

Pancreas

The pancreas is a composite gland — both exocrine and endocrine. Its endocrine part is the Islets of Langerhans: about 1–2 million islets, only 1–2% of pancreatic tissue. The main cells are α-cells (secrete glucagon) and β-cells (secrete insulin). Glucagon, a peptide hormone, acts mainly on liver cells (hepatocytes): stimulates glycogenolysis and gluconeogenesis and reduces cellular glucose uptake → hyperglycemia — a hyperglycemic hormone. Insulin, a peptide hormone, acts mainly on hepatocytes and adipocytes: enhances cellular glucose uptake and glycogenesis → hypoglycemia. Glucose homeostasis is maintained jointly by insulin and glucagon. Prolonged hyperglycemia → diabetes mellitus, with loss of glucose in urine and formation of harmful ketone bodies; treated with insulin therapy. Pancreas = composite. 19.2.9TestisवृषणTestis

A pair of testes lies in the scrotal sac (outside the abdomen). Each testis has seminiferous tubules and stromal (interstitial) tissue. The Leydig (interstitial) cells in the intertubular spaces produce androgens, mainly testosterone. Androgens regulate development, maturation and functions of the male accessory sex organs (epididymis, vas deferens, seminal vesicles, prostate, urethra, etc.), stimulate muscular growth, growth of facial and axillary hair, aggressiveness and low pitch of voice, play a stimulatory role in spermatogenesis, act on the CNS to influence male sexual behaviour (libido), and have anabolic effects on protein and carbohydrate metabolism. Testis (scrotum) → Leydig cells → androgens (testosterone): male accessory organs, muscles, facial hair, aggressiveness, bhaari awaaz, spermatogenesis, libido, anabolic effects.

Ovary

A pair of ovaries lies in the abdomen. The ovary produces one ovum per menstrual cycle and two groups of steroid hormones: estrogen and progesterone. It has ovarian follicles and stromal tissue. Estrogen is secreted mainly by growing ovarian follicles; after ovulation the ruptured follicle becomes the corpus luteum, which secretes mainly progesterone. Estrogens stimulate growth and activities of female secondary sex organs, development of growing ovarian follicles, appearance of female secondary sex characters (e.g., high pitch of voice), mammary gland development and female sexual behaviour. Progesterone supports pregnancy, and acts on mammary glands to form alveoli (sac-like structures that store milk) and stimulate milk secretion. Ovary (abdomen) → estrogen (growing follicles): female sex organs, secondary characters, mammary development. Corpus luteum → progesterone: pregnancy support, mammary alveoli, milk secretion.

Hormone kaise kaam karte hain

Hormones act by binding to specific receptors present only in target tissues. Receptors on the cell membrane are membrane-bound receptors; those inside the cell are intracellular receptors, mostly nuclear receptors. Binding forms a hormone–receptor complex, causing biochemical changes in the target tissue. On the basis of chemical nature hormones are: (i) peptide, polypeptide, protein hormones (insulin, glucagon, pituitary and hypothalamic hormones), (ii) steroids (cortisol, testosterone, estradiol, progesterone), (iii) iodothyronines (thyroid hormones), (iv) amino-acid derivatives (epinephrine). Hormones on membrane-bound receptors usually do not enter the cell but generate second messengers (cyclic AMP, IP₃, Ca²⁺) which regulate cellular metabolism. Hormones on intracellular receptors (steroid hormones, iodothyronines) mostly regulate gene expression or chromosome function through the hormone–receptor complex interacting with the genome. Hormone + specific receptor = complex. See hormone action diagram (a). FSH is a protein (glyco-protein) hormone of the anterior pituitary, so it does not enter its target cells. It binds specific membrane-bound receptors on target cells in the gonads, forming a hormone–receptor complex. This generates a second messenger (cyclic AMP) inside the cell, which regulates cellular metabolism and brings the response: in females growth and development of ovarian follicles; in males, together with androgens, regulation of spermatogenesis. FSH = protein hormone → membrane receptor → cAMP (second messenger) → female: follicle growth; male: spermatogenesis.
